Formado en Boston, Extreme está compuesto por Gary Cherone, Nuno Bettencourt, Pat Badger y Paul Geary. Aunque Cherone y Geary ya se conocían y habían trabajo juntos desde el '83 con la banda The Dream, fue la llegada de Bettencourt y Badger la que dio forma a la banda. Firmaron un contrato con la compañía discográfica A&M y finalmente, en 1989, sacaron su primer disco llamado como ellos mismos. En él se incluían temas como "Play With Me". Mezclando elementos Pop, Metal, Funk y Blues, el disco pasó desapercibido y los críticos se mostraron indiferentes. En 1990, sacaron su segundo trabajo "Pornograffitti". "Get The Funk Out" alcanzó el puesto 19 en las listas inglesas, pero fue la sencilla balada acústica "More Than Words" la que alcanzó el segundo puesto y fue número uno en América. El tema "Hole Hearted" consiguió también una buena posición en las listas de éxitos. La música del grupo estaba caracterizada por la innovadora forma de tocar de Bettencourt, sus inteligentes letras y la influencia de muchos estilos musicales. Su tercer álbum fue "III Sides To Every Story" y con él, Extreme se consolidó como un gran grupo. En 1994, Paul Geary dejó el grupo y Mike Mangini se unió a la banda. Con está nueva formación, un año más tarde lanzaron el disco "Waiting For The Punchline", que no tuvo el éxito esperado y recibió numerosas críticas negativas. Bettencourt anunció que abandonaba el grupo para iniciar una carrera en solitario y finalmente, en octubre de 1996, Extreme se disolvió, yendo Cherone a trabajar con Van Halen (dejando este grupo en 1999).
